Everytime I had this kind of problem it was the spool brake or the spool gripper that not grip the inside of the paper core correctly.

If it's happen near the end or less than 25% of paper on the roll than it could be because the paper is not tight enough or not stick on the core itself. The paper just spin around the core in that case.

That was the first thin i thought about it being the brake but i had a brand new spindle and it is still doing it. And doesn't matter how much is on the core at all.

Double check the "bite" of the spindle on the inside of the paper core. If it slips during paper check, it will unload the paper.
