I just had this issue on a 3000. Refer to TB-K105-019 Roll Spindle moves upwards near end.pdf. This fixed our problem. What happens is the roll is too loose in the drawer and it throws the sensor timing off, then feeds the roll out on the floor.

Good Luck

CJWILT is correct. Ever notice at the end of a roll, the machine will usually spit out the last couple feet of blank paper. That is because the sensors sense that there is paper in the paper path but no longer movement on the spindle, therefore it cleanse itself by extracting the leftover paper. Same concept, except that the paper spindle gear isn't detecting paper movement because the spindle has lifted off the gear/sensor, therefore the machine says," I'm going to spit out the rest of this paper" and it doesnt' stop until there is no more paper detected in the paper path, hence a nice heap of paper in front of the printer.
